The USPC is only related to UnitedHealthcare's individual line of business and, specifically, only the Program Plans, which are those health insurance products for individuals and families underwritten and/or administered by Golden Rule Insurance Company, including Short Term Medical, and Medicare Supplement plans. USPC membership is limited to brokers with an in-force Golden Rule Insurance Company Independent Broker's contract.

To join the program, the broker must pay a one-time $100 enrollment fee that is due at the time of enrollment into the program.

You can choose from two programs:

  1. Cash Bonus Option
    • $50 cash bonus will be paid to the broker per issued and placed application.
  2. Norvax products/services available to USPC members are:
    • A subscription to the Golden Rule - sponsored Norvax Technology suite (consisting of a single carrier QuoteBuilder, five response LeadMiner package, basic one page Web site, and the Broker Office Lead Management System or upgrades) equaling at least Three Thousand Dollars ($3,000) in annualized fees.
    • Any and all necessary education, training, and support for effective utilization of the items listed above.
    • Fifty Dollars ($50) Norvax credits toward the purchase of (1)advance Norvax technology, including customizable websites, LeadMiner Marketer autoresponders, co-browsing tool and/or BrokerDialer. If the total cost of the advanced Norvax technology is not fully covered by the broker's production, the the rest of the monthly bill is expected to be paid for by the broker. (2)Health Internet Leads from broker's choice of at least three different Internet-based lead vendors and one telemarketing lead vendor. All lead transactions shall occur via a Web site maintained by Norvax regardless of whether Norvax or another vendor provides the leads. (3)The tickets costs into any Norvax University Conferences. This only includes what the tickets cover, which does not include airfare, hotel and certain meals. Each conference covers slightly different aspects of the conference so this will vary according to what the conference tickets covers per event. (4)Keep the Norvax Bucks to create a bank that Brokers can use at a later date. The Norvax Bucks are earned through each Program Plan Issued and Placed while broker is a USPC member.

However, brokers who are already receiving any of the above products/services from Norvax as of the date of their enrollment in USPC shall not receive the Norvax products/services above, but instead, they shall only receive lead credits specified in C. above plus Norvax technology upgrades of their choice equaling at least Three Thousand Dollars ($3,000) in annualized fees.

USPC members may contract directly with Norvax to receive, at their own expense and in accordance with Norvax's usual business practices, products/services in addition to or different than those described above.

"Issued and Placed" means an insurance application for a Program Plan that lists a USPC member as Broker of Record, and in response to which coverage is issued, coverage is accepted by the primary insured, and coverage remains in force for at least 30 days.

ELIGIBILITY

Issued Short Term MedicalSM and Standalone Dental count toward the USPC trip and quarterly bonuses. Beginning July 1, 2009, the trip and quarterly bonus qualifications will be calculated on a new point system. The following points will be awarded for each issued Individual Health, Short Term, and Standalone Dental Application.

Points per issued Product:

HSA 100 equals ten (10) points.
HSA 70 equals ten (10) points.
Individual Health Application equals six (6) points.
IMG equals one (1) point.
Short Term Application (with policy duration of 1-6 months) equals one (1) point.
Short Term Application (with policy duration of 7-12 months) equals two (2) points.
Standalone Dental Application equals one (1) point.

Quarterly Bonus:

The following points are needed to reach the various calendar quarterly bonus levels for quarters 3 and 4 of 2009.

250 points equals $5,000.
220 points equals $4,500.
190 points equals $3,500.
160 points equals $2,500.
130 points equals $2,000.
100 points equals $1,500.
  75 points equals $1,000.

Trip Details:

You and a guest could be on your way to the luxurious Stowe Mountain Resport. To qualify for this exclusive, luxury trip you must earn 200 points between January 1 and June 30, 2010. The trip takes place October 15-18, 2010.

Brokers must be licensed with Golden Rule Insurance Company and a member of the United Select Producers Club (USPC) to be eligible to participate. Applications must be received between January 1, 2010, and March 31, 2010, and issued by April 16, 2010, for the 1st quarter bonus. Applications must be received between April 1 and June 30, 2010, and issued by July 16, 2010, for the 3rd quarter bonus. Bonuses are not cumulative; limit one bonus per quarter per broker. To qualify for the trip for two, 200 points must be reached by submitting business from the time period of January 1, 2010, through June 30, 2010, and the business must be issued by July 16, 2010. Retail value of trip is taxable as income and will be reported to you on an IRS 1099MISC (assignment of commission does not apply). Trip is not transferable. Trip and bonuses will be awarded to initial soliciting agent noted on application, and will not be affected by any subsequent change. The trip will be provided by an independent travel arranger, and Golden Rule is not responsible for that arranger's performance. Trip is subject to availability; other destinations may be substituted solely at Golden Rule's discretion. Trip is not all expenses paid. Applications must be new clients only; rewrite of existing UnitedHealthcare Individual Line of Business clients does not count toward program with the exception of consecutive Short Term Medical plans. Points are awarded per application regardless of number of insureds, and any separate applications of family members count as only one application for program. Eligible products included Golden Rule health plans for individual and families except Medicare Supplement. Results are based on individual broker production only. All individual health plans must be in force for a minimum of six consecutive months after the effective date in order to count as "issued". Applications cannot be split between brokers. Program open to select brokers only. We reserve the right to make final judgment on program qualifiers including, but not limited to, disqualifying brokers who fail to act in best interests of their clients. Any required disclosures to broker's clients are broker's sole responsibility.

Bonuses are based on individual USPC member production only. Bonuses are not transferable. Bonuses are not cumulative; limit one calendar quarter bonus per USPC member. Assignment of commissions does not apply. Applications cannot be split between USPC members or between a USPC member and any other broker. Any separate applications of family members count only as one application for contest purposes.

GENERAL

Golden Rule and Norvax both independently reserve the right to discontinue or modify the USPC or contests at any time without notice.

Norvax and Golden Rule (including each of its affiliated companies) are separate and distinct entities. Golden Rule does not endorse Norvax or its products and is not responsible for Norvax's performance. The trip is provided by an independent travel arranger, and Golden Rule is not responsible for that arranger's performance. Trip is subject to availability; other destinations may be substituted solely at Golden Rule's discretion. Trip is not all expense paid. 1099s apply. Golden Rule reserves the right to make final judgment on contest bonus recipients. Any required disclosures to brokers' clients are brokers' sole responsibility.
